This appointment is for a consultation (in person at WILD) for 1 pet who needs food therapy to address health issues. After you schedule this appointment, you will receive a questionnaire via email (coming from IntakeQ). Please complete this and submit back to us 24 hours prior to your appointment time. If you do not receive these forms, please check your spam folder and/or email us to resend.

Cancellation Policy

To cancel or reschedule, please contact us with at least 24 hours notice to avoid a $60 cancellation fee.
