After you schedule this appointment, you will receive an questionnaire via email. Please complete this (as well as uploading any recent vet records & lab results) and send back to us via email.

Cancellation Policy

To cancel or reschedule, please contact us with at least 24 hours notice to avoid a $60 cancellation fee.
