For established nutrition clients whose food plan needs a reassessment and it's been within 6 months since your last appointment. This appointment is for one pet. After you schedule this appointment, you will receive a questionnaire from IntakeQ via email. Please complete this and submit back to us 24 hours prior to your appointment.

Cancellation Policy

To cancel or reschedule, please contact us with at least 24 hours notice to avoid a $60 cancellation fee.
